Lean Six Sigma Master Black Belt Training Program Information and Requirements

Requirements for Participants:

• Lean Six Sigma Master Black Belt training participants are responsible for their own transportation, lodging and meals.

• The participant is required to bring a notebook computer to the training loaded with Minitab statistical software.

• Registrants need to purchase the course textbook, Implementing Six Sigma, by Forrest W. Breyfogle III, Second Edition, separately and bring it to the course with them.  Implementing Six Sigma is the premier Lean Six Sigma textbook on the market.  It doubles as a Lean Six Sigma handbook.

• Some previous experience or training in Lean or Six Sigma, quality, process improvement, or related fields is usually expected, but not required.

Participants need to obtain Minitab for use in class.  A free download, lease, or purchase of Minitab software is available from Minitab. Please have it loaded on the notebook computer used during the course.  Practicing with it a little before the course date is recommended.  Many of the demonstrations and examples presented during the course will utilize Minitab.

• Attire:  Business casual or casual attire is recommended for all Lean Six Sigma training sessions.

Six Sigma Partnering Provides:

• Daily coffee, soda and snacks in the conference room.

• Six Sigma Partnering LLC will select the hotel and notify each Registrant at least one month in advance of the training date. Hotel registration and contact information will be provided at that time. The information will be updated to this web page as well.

• Six Sigma Partnering LLC provides each attendee with a digital copy of the slide presentation and Six Sigma Partnering Six Sigma Black Belt Tools Software.  In addition, each participant will receive a handy pocket reference text.

• Upon successful training program completion, the participant shall be granted certification as a Lean Six Sigma Master Black Belt, by Six Sigma Partnering LLC.  The participant will receive an 8x10 certificate suitable for framing and their name will be prominently displayed at the Graduate Registry website page.

Cancellation Policy:

• Six Sigma Partnering LLC may cancel or reschedule any course at any time with 30 days notice and is not liable for any special or consequential airfare penalties or other travel charges that may occur as a result of such action.  The client will be rescheduled in a subsequent training session or a full 100% refund of the registration fee will be made.

• Client initiated cancellation charges will be assessed in accordance with the following schedule:

Warranty:

Six Sigma Partnering LLC offers no warranty, expressed or implied, that this training will lead to career advancement or increased income for the participant.
 

License Agreement:

The registered participant agrees that the Licensed Materials obtained from Six Sigma Partnering LLC are for personal use only and are forbidden from resale or distribution without the expressed written consent of Six Sigma Partnering. The Licensed Materials include:  Six Sigma Partnering Software Tools and other software based solutions or documents provided throughout the training program.  These tools are offered for the participant's use in an "as is" condition.

Liability Statement:

Six Sigma Partnering LLC shall not be liable for any indirect, special, incidental, punitive or consequential damages, including but not limited to loss of data, business interruption, or loss of profits, arising out of the use of or the inability to use the Licensed Materials or due to the cancellation of a training session.

Six Sigma Partnering LLC makes no representation or warranty, and expressly disclaims any liability with respect to the content of any Licensed Materials, including but not limited to errors or omissions contained therein, libel, infringement of rights of publicity, privacy, trademark rights, moral rights, or the disclosure of confidential information.

Except for the express warranties stated herein, the Licensed Materials are provided on an "as is" basis, and Six Sigma Partnering disclaims any and all other warranties, conditions, or representations (express, implied, oral or written), relating to the Licensed Materials or any part thereof, including, without limitation, any and all implied warranties of quality, performance, merchantability or fitness for a particular purpose. Six Sigma Partnering makes no warranties respecting any harm that may be caused by the transmission of a computer virus, worm, time bomb, logic bomb or other such computer program.
